What happens in an Individual Nutrition Counseling Session?
Initial Nutrition Counseling Session (75 min)
Once you book an initial session, I will have you fill out a new client intake form, which includes some questions about your medical history, typical eating patterns, lifestyle habits, and your primary nutrition concerns. This information will be used as a basis for the initial session in which we will discuss some of the information in further detail as well as any other factors that may influence your eating behaviors or thoughts about food. We will then discuss goals and the steps that will be involved in getting there.
I truly believe every individual is unique and will take my time to really get to know you in order to find the best way for your body, what works your lifestyle and what leads to your happiness.
Follow-up nutrition counseling Sessions(45-50min)
Follow-up sessions will focus on discussing your progress since the previous session, so we can find out what is working for you or not in order to tailor our strategies toward your goals. They will also involve nutrition education, counseling for lifestyle issues influencing eating and thoughts regarding food/body as well as setting next-steps. Session Fees and Information Regarding Insurance
Initial session (75 min): $195
Follow-up sessions (45-50min): $125
* I offer discounted rates for former clients from Seattle Pacific University during the summer months and/or after graduation. Contact me at email@example.com for details.
* Limited spots for sliding-scale fees are available if financial restrictions are a barrier in pursuing nutrition counseling. Please contact me to discuss.
METHODS OF PAYMENT
I accept cash, check, all major credit cards and HSA(Health Savings Account), FSA(Flexible Spending Account) cards.
**Please note that this may not apply to on-line nutrition coaching sessions.
Please note that I am a out-of-network for all insurances, therefore payment is due at time of service. However, there is still a good chance that your insurance company will provide partial or total coverage for out-of-network services depending on your benefits. As nutrition counseling & medical nutrition therapy benefits with insurance companies greatly vary, please speak with your insurance company representative to find out what your benefits may cover in working with me. If your coverage qualifies for reimbursement, I can provide you with a detailed invoice with my NPI(National Provider Identification) number that you can submit to your insurance company.
Here are some questions to ask your insurance company to find out about your out-of-network nutrition counseling benefits:
Do I have out-of-network nutrition counseling/Medical Nutrition Therapy benefits?
Is there a deductible that I need to meet?
Are there a limited number of visits per calendar year?
What is my percentage of coverage? Is there a co-pay?
Do I need a diagnosis and referral from my primary care provider?
I often see that clients who make the time, effort, and financial commitment are able to build a stronger motivation to health and well-being and it also allows us the therapeutic freedom in nutrition counseling to reach your goals without limits to session length or frequency of visits. However, I if financial restrictions are your only barrier in seeking nutrition counseling and you are highly motivated to work on your nutrition/food/body concerns, please reach out to me to discuss sliding scale fees that are available on a limited case-by-case basis.
All changes to scheduled appointments need to be made at least 48 hours in advance of the scheduled session to avoid being charged for the session. Missed appointments or late cancellations will be charged for the full session. If you arrive to your appointment late, the session will end at the scheduled time regardless of when it started and full payment is expected. I understand that at times life happens and when situation permits, I may see you for the full session. However, as a courtesy, please let me know when situations arise as soon as possible in order to help me to serve you and other clients better by keeping scheduled appointments.
